Directions for New Patients

Welcome to our office! To help make your first visit as smooth and efficient as possible, please review the instructions below before your appointment.

1. Complete Your New Patient Forms
Please print out and complete all new patient forms prior to your visit. This helps us review your medical history and focus on your concerns during the appointment. If you are unable to complete them in advance, please arrive 15-20 minutes early to allow time to fill them out in the office.

2. Bring Your Insurance and Identification
Please bring:

• A valid photo ID

• Your insurance card(s)

• Any required referral from your primary care physician (if applicable)

 

3. Have a List of Your Medications Available
Please bring a list of all medications you currently take, including:

• Prescription medications

• Over-the-counter medications

• Vitamins and supplements

• Inhalers, nasal sprays, or allergy medications

 

4. Bring Relevant Medical Records (If Available)
If you have had prior allergy testing, breathing tests, imaging studies, or relevant specialist evaluations, please bring copies of those records to your appointment if possible.
